Report on pilot test of Personal Health Care Portfolios as part of the Person-Centered Health Care Supports Project. For six months, eleven adults with developmental disabilities took part in this research project, developing health portfolios about themselves and recording how they used them to determine if the portfolio model was an effective health communication tool.
