Establishing a Flow of Services to Strengthen Local Collaboration

This implementation brief describes how the project uses the Flow of Services (FoS) tool to strengthen coordination among schools, vocational rehabilitation, Centers for Independent Living, families, and community partners supporting youth with disabilities. The brief explains how Local Interagency Teams customized the tool to map services, clarify partner roles, identify service gaps, and improve collaboration to support successful transitions to employment, postsecondary education, and independent living.
