The Partnerships for Progress in Colorado project aims to enhance transition services for youth with disabilities by fostering strong, lasting partnerships. Over five years, the project will introduce new approaches to help families, youth, and service providers plan and coordinate services. Its two main goals are: 1) creating an online platform to share best practices and support statewide collaboration, and 2) building community resources to improve access to quality services. The project focuses on underserved youth while improving services for all youth with disabilities. Key partners include universities, Colorado Division of Vocational Rehabilitation, and other disability-focused organizations. Data will be used to improve outcomes.
