Title Person-Centered Practices Self-Assessment
Summary This Self-Assessment is designed to help leadership at human service agencies in States, Tribes, and Territories to measure their progress in developing a more person-centered system. It is designed for use by the administrative entities (“agencies”) with the authority to administer or operate federal programs for long-term services and supports, including home and community-based services. It can be used by the wide range of systems that oversee services and supports for people with disabilities of all ages, older adults with long-term service and support needs, and other health and social service programs.
